Definitions for hematologic response in AL amyloidosis

Response categoriesDefinitions
Complete responsea Negative serum and urine immunofixation electrophoresis
and normalization of FLC ratio (or uninvolved FLC concentration greater than involved FLC concentration with or without an abnormal FLC ratio)
Very good partial response If baseline dFLC ≥50  mg/L: reduction in dFLC <40  mg/L
Partial response If baseline dFLC ≥50  mg/L: reduction in dFLC >50%
If baseline dFLC 20-50  mg/L: reduction in dFLC <10  mg/L
No response All other patients

In the ANDROMEDA study,34 a reduction in the absolute involved free light chain level less than or equal to 20  mg/L or a dFLC level less than 10  mg/L were also considered an excellent hematologic response.
