Clinical criteria for the diagnosis of acute lung injury (ALI) and transfusion-related ALI (TRALI).35 

*For patients without an arterial blood gas, pulse oximetry less than 90% meets the criteria for hypoxemia. 
Irrespective of the pulmonary end expiratory pressure (PEEP). 
  1. Insidious, acute onset of pulmonary insufficiency

  2. Profound hypoxemia PaO2/FiO2 < 300 mmHg*

  3. Chest radiograph:

    • bilateral fluffy infiltrates consistent with pulmonary edema

  4. Cardiac:

    • Pulmonary artery wedge pressure ≤ 18 mm Hg

    • No clinical evidence of left atrial hypertension
