Ifosfamide: 5 g/m² mixed with an equal amount of mesna and administered over 24 hours. Carboplatin: dosed at an area under the curve of 5 (5× [creatinine clearance + 25]) and capped at 800 mg, administered as a bolus. Etoposide: 100 mg/m², administered as a bolus. G-CSF: 5 μg/kg for the first two cycles, then 10 μg/kg beginning on day 5 of the third cycle and continuing through leukapheresis. Cycles were to be administered every 14 days, for a total of 3 cycles.
