On page 3077, in the last row of Table 1 [Subsequent malignancy (≥5 y postdiagnosis)], the number reported (1447 [82.2%]) is actually the number of surviving patients without a subsequent malignancy. The number and percentage of survivors with a subsequent malignancy should be 313 (17.8%).
